What information is needed to apply?

Working to gather any and all medical evidence to support your claim ahead of time will be additionally helpful. Plan to have your Social Security number and proof of age. You will need the names, addresses and phone numbers of the hospital(s) and/or clinics, doctors and caseworkers that assisted you during your treatment, as well as the names and dosages of all medications prescribed during those visits. Have medical records and laboratory test results in your possession at the time you apply, as well as your employer’s information. A copy of your most recent W-2 is also needed, or federal tax return if you are self-employed.

After your application is received, you will receive a confirmation of your application, either electronically or by mail. During the review of your application, you may be contacted if more information is needed. When your application is processed, the decision will be mailed to you.
