You must have been a worker that used, prepared, or had been exposed to a paraquat product applied by a licensed applicator after 1961. Examples include farmers and farm workers, herbicide applicators, chemical mixers, flaggers, tank fillers, people living adjacent to fields where paraquat was used, and other workers who mixed and/or applied paraquat under the supervision of a licensed applicator prior to 2016. The use cannot be residential. It must be commercial.
